The following forms give me valuable information.  Please take time to give them your careful consideration.

• Student Information Form: Basic information about how to get in touch with you, why you are taking the course, and how comfortable you are with the use of technology.
• Spotcheck: Send the instructor criticism or recommendations about anything related to the course at any point in the term. I check these periodically.
• Course Evaluation Form: Use this form to evaluate the course.
• Computer Utilization Survey: Feedback on your experience of technology use with this class (available toward the end of the term).
